Why Participate
Your answers and feedback will be part of the New Haven Health Department’s citywide community health assessment (CHA), a project to better understand the state of public health in our city. Results from the CHA will inform the community health improvement plan (CHIP), a guide for deciding on our funding, program, and collaboration efforts for the next three to five years.
